SSDR fixes

The patch updated the Discipline Incident Report to DOE screen.

We have made the Description and Comment boxes smaller.

We also removed an extra Offense Description box.

The original “Additional Description for Offense Type” box is being renamed “Additional Description of Offense Type 1.”

Any text that was entered into the extra/new “Description for Offense Type 1” box that has been removed was moved for you to the updated “Additional Description of Offense Type 1” box.
